A marriage certificate is a document that lists the full names, addresses, and birthdates of both spouses, as well as the date and location of the marriage. This document is signed by the officiant and two witnesses, and is then filed with the government. 2. A copy of your identification

3. A copy of your spouse's identification

4. A completed apostille application form
