A death certificate is a document that lists the cause, location, and time of someone's death. It also includes the person's full name, date of birth, and other identifying information. An apostille is a certificate attached to a document that authenticates the document's origin and verifies that it has been signed by a person authorized to do so.
If you plan to move to another country, you may need to get your death certificate apostilled. An apostille is a certified copy of a document that has been authenticated by the issuing authority. In order to be accepted in another country, your death certificate will need to be apostilled. The Division of Corporations in the Department of State is the only office that can issue an Apostille for documents originating from New York State.
The Apostille Certificate verifies the authenticity of the document's signatures and notarizations. It is a certificate that goes on the back side of the document and, once affixed, the document becomes legally recognized in countries that are members of the Hague Convention.
If you need an Apostille Certificate for a document originating from New York State, you must submit the following to the Division of Corporations:
- The original document that you need authenticated. If the document is in a foreign language, you must also submit a certified English translation.
- A completed Apostille Certificate Request Form (notarized).
- The appropriate fee ($10 for the first Apostille Certificate and $4 for each additional Apostille Certificate on the same document). Fees must be paid by check or money order made payable to the "Department of State". Apostilles are typically issued for official documents such as Birth Certificates, Marriage Certificates, Death Certificates, Power of Attorney, and educational documents like GED's, diplomas and transcripts.
In order to get an apostille for a death certificate, you need to contact the New York State Health Department. The Health Department will then send you a form to fill out and submit. After the form is submitted, the Health Department will verify the information and send you an apostille.
